This initiative will kick-off on Apri 1, 2025 and conclude on April 30, 2025. The program is designed to address health disparities impacting underserved African-American communities, and we believe that with your support, we can make a meaningful difference.

April is National Minority Health Month. Our mission is to empower residents by providing essential health education and resources that contribute to improved health outcomes. The "Healthy Futures 2025" program will focus on increasing awareness around chronic diseases and mental health while promoting healthy lifestyles and fostering community support. Here are some of our key objectives:

Dear Volunteers,
Thank you so much for your interest in engaging with our participants! We truly appreciate your willingness to contribute in many different ways. As a dedicated volunteer healthcare provider, you have the wonderful opportunity to coordinate health screenings in person and also host online informational workshops on various healthcare topics.
This flexibility also applies to our nutritionists and fitness enthusiasts. You can choose to offer a combination of in-person classes and online sessions, creating a hybrid approach that suits your schedule and the needs of our community.
To get started, we kindly ask each volunteer—whether you plan to participate in person or online—to submit your availability, the topics you would like to cover, and the chosen format (in-person or online) to our organizer at FCN Medical Group. We will then compile this information to create a calendar of events for the month of April, which will be shared with our participants through print advertisements, radio and television, as well as social media routes like Instagram and Facebook.
It’s also important to note that, in addition to gathering volunteers for these impactful workshops, we need your support through monetary donations. These contributions will help us secure the necessary resources to spread valuable information to underserved, minority, and needful communities.

OBJECTIVES

The program has several key objectives aimed at improving health in African-American communities. First, it seeks to raise awareness by educating community members about the significant health disparities they face, including chronic diseases, mental health challenges, and barriers to accessing healthcare. Second, the program aims to promote healthy lifestyles by encouraging behavior change through initiatives focused on nutrition education, physical activity, and mental wellness.

In addition, the program plans to build community resources by establishing partnerships with local healthcare providers, organizations, and schools. This will facilitate better access to healthcare services and resources. Finally, the program strives to foster community engagement by creating an inclusive environment where community members can participate in health discussions, share their experiences, and support each other.

COMPONENTS
​
Community Health Workshops
The program will offer workshops focused on various important health topics, including chronic disease management (such as diabetes, hypertension, and obesity), mental health awareness, maternal and child health, and preventative care. These workshops will take an interactive format, featuring guest speakers, live demonstrations, and question-and-answer segments. They will be held monthly at community centers, schools, or places of worship.
Nutrition and Cooking Classes
We will organize nutrition and cooking classes that emphasize the preparation of affordable, nutritious meals featuring traditional foods. The classes will focus on essential cooking techniques, meal planning, and budgeting skills. Participants will engage in hands-on cooking demonstrations, taste testing, and recipe sharing. To enhance these classes, we will collaborate with local farmers’ markets and food banks to provide fresh produce.
Physical Activity Initiatives
To encourage active lifestyles, we will establish various physical activity programs, such as walking groups, fitness challenges, and sports leagues. Community health fairs will feature fitness demonstrations, Zumba classes, and yoga sessions. To motivate participation, we will provide incentives, such as gym memberships, fitness gear, or health screenings.
Mental Health and Wellness Programs
Our mental health and wellness programs will include workshops focused on stress management, emotional wellness, and resources for mental health support. We will create support groups that provide safe spaces for discussing mental health challenges, facilitated by licensed professionals. Additionally, we will develop resource guides that inform participants about local mental health services and counseling options.
Health Screening and Referral Events
The program will host health screening events that offer free or low-cost assessments for blood pressure, cholesterol, diabetes, and BMI. Participants will be referred to local healthcare providers for follow-up care as needed. We will collaborate with local clinics and pharmacies to facilitate these screening events.
Community Advocacy and Education Campaign
We will launch advocacy and education campaigns to raise awareness about specific health issues, such as hypertension. Our outreach efforts will include creating informational pamphlets, running social media campaigns, and broadcasting segments on local radio to share health information. Success stories from community members who have made positive lifestyle changes will be highlighted to inspire others.
Evaluation and Feedback

To assess the effectiveness of the program, we will conduct pre- and post-program surveys to measure knowledge gains, behavior changes, and participant satisfaction. Focus groups will be organized to gather qualitative feedback and suggestions for improvement.
